REFERRING PHYSICIANS
Physicians or health professionals can send referrals by faxing to 866.282.9069.
Referrals should include:
Patient Demographics
Copies of diagnostic reports, images and progress reports
Prior Authorization if applicable
Copies of both sides of the patient's insurance card
